Concordia Students: An additional Concordia Tri-College Application form is located on the Concordia Registrar's webpage. The Concordia form + Tri-College Registration form is required for all Concordia students.

Complete the Form

PDF forms are the best way for the 5 registrar's offices to process forms between the campuses and save student records.

Understanding this is a manual process, it is critical you submit a "near perfect" form attached to a detailed email that includes any additional information. 

A carefully completed form always speeds up the review process at the home campus registrar's office.

 

Use this checklist for completing forms:

  • Type the form.
  • Save the form to your computer. Save time and frustration - forms are too long to attempt on a smart phone.
  • Complete every box because it's required information for the 2 registrar's offices and for state/federal compliance. If not applicable, mark "n/a."
  • Help for:  Find the Correct Number on the Course Schedule   How to Register for Alternate Sections of a Class   How to Register for an Alternate Class.
  • Proof.Proof.Proof. Ask a friend to proof. This will eliminate time consuming email exchanges with the registrar's offices.
  • Use campus directory link to email instructor for permission. Keep in mind, instructor permission is not a guarantee of getting a seat in the class. 
  • Use ONLY official campus email to send forms and communicate with the registrar's offices. 
     

Registration Form Sections

Box 1.   The 2 Campuses involved in the registration request. 
Box 2.   Personal data to create your student record at the host institution.
Box 3.   Citizenship/Demographic information required for federal compliance.
Box 4.   Class information so the host campus can process your request.
Box 5.   Your signature represents that you accept academic and financial responsibility for the class.

Email the Forms(s)

From:  Your campus email

  1. Attach completed and proofed form(s). 
  2. Attach the email from the instructor if permission is required. 
  3. If you are not eligible, attach the completed appeal form - advisor's signature required.
  4. In the body of your email explain your request and attached documents. 
  5. CC yourself on the message and keep track of your email for future reference. 
  6. SEND. When possible, send one email with all required attachments. 
  7. Watch your email closely for questions from the registerar's office and respond promptly ONLY from your campus email. 
     

Timeline for Processing

Phase 1 - Home Campus Review

Allow up to 10 business days for review and notification via email. In general, this is what is being reviewed:

  1. Is the form complete?
  2. Are you enrolled? 
  3. Are you eligible? If not, is the appeal attached?
  4. Do you meet the host campus prerequisites and co-requisites? 
  5. Does the class satisfy a graduation requirement? While not a requirement, the registrar's office wants to make sure you are informed. 
  6. Is instructor permission required? If yes, is the email attached?

 

Phase 2 - Host Campus

Fully reviewed forms are forwarded to the host campus and date stamped.

  1. AT OPEN ENROLLMENT, forms are processed in the order received. 
  2. Go to theTri-College combined calendar and mark the open enrollment date on your calendar, as this is the earliest possible date you will hear about your registration request. 
    • To be clear, the host campus registrar's office will not process Tri-College registrations before home campus students have completed registration. 
  3. If class space is not available, you will receive an email. (HINT: Add the host campus registrar's email to your Outlook contacts so you'll quickly see it.)
  4. If there is space in the class, you will receive an email that your registration is processed.  
    • You are added as a student at the host campus. 
    • The host campus email includes the information and steps you need to complete to set up your student account.
    • The home campus registrar's office is copied on the email, and the class is added to your student record. 
  5. Until you receive registration confirmation, you may not attend class unless given specific approval by the host campus registrar's office.

 

Troubleshooting PDF Forms

Problem: completed form is empty or some fields are empty.  Possible solutions:

Problem: Mac users report that emailed forms are empty when received by the Registrar's Office. Possible solutions:

  • Complete the PDF form in Preview > choose print from the file menu > use the dialog box to "save to PDF."
  • Use Adobe Reader software instead of Preview. 

Problem: using the Adobe Reader Mobile App. Possible solution: 
Use your computer - the Acrobat Reader mobile app has a limited number of tools and features.
